Addison County Home Health and Hospice
Addison County Home Health and Hospice, Inc. (ACHHH) was founded in 1968 by a committed group of Addison County residents to meet the county’s need for community based professional home care services. ACHHH offers a broad spectrum of home care services to improve quality of life and support people in maintaining their independence at home. Our services are available to families and individuals of all ages, regardless of the extent of their illness or injury.
Governed by a volunteer Board of Directors, ACHHH is a non-profit Medicare certified provider of comprehensive home care services. Professional services include: skilled nursing, physical therapy, speech therapy, occupational therapy, medical social work, and nutritional consultation. Ancillary support services include home health aide, personal care attendant, and homemaker.

Home care services are provided under the direction of your physician. That means your doctor must refer you for you to qualify for services. After your doctor refers you, a nurse or therapist visits in your home to discuss and assess your needs. The nurse or therapist works with you and your doctor to develop your plan of care.
